PCC's Hepatitis C Program offers complex and integrative treatment of viral hepatitis. The program has a multidisciplinary team including physicians, nurse practitioners, specialty pharmacists and clinical nurses with expert knowledge in the treatment and management of Hepatitis C.

The program is the first of its kind to offer comprehensive services using a collaborative and personalized approach to treatment of viral hepatitis C. We integrate behavioral health, primary care and substance use treatment when needed. PCC's dedicated team can screen, treat, and manage patients with Hepatitis C all within our clinic using national treatment guidelines without the need for referrals. Our program is designed with you in mind. Our goal is to reduce treatment barriers by using our partners to provide expert, comprehensive care throughout the treatment process, making sure you feel supported every step of the way.

Patients will be referred to the Hepatitis C Program by their primary care provider. The patient is then added to our treatment list and contacted by program staff. The patient will undergo an intake process that includes labs, imaging, counseling, and education. During intake, follow-up appointments are planned. Once the patient is approved for the program, they are closely monitored by our program staff. Our program is designed to be comprehensive, patient-centered care with an extensive team that includes: HCV Program Coordinator, HCV RN, Specialty PharmD, HCV provider, behavioral health support, and case management support by ECHO-Chicago.

Family planning, reproductive health, and family building services are a vital part of our primary care model. PCC strives to help patients reach their reproductive goals by providing holistic, sensitive, and gender-affirming reproductive and sexual healthcare services in a supportive environment. Our services include:

·       Confidential services for youth age 12+

·       All-options pregnancy testing and counseling

·       Birth control, including management of period and menstrual cycle related concerns such as painful or heavy periods or premenstrual dysphoric disorder

·       Emergency contraception

·       Sexually transmitted infection (STI) testing, counseling, and treatment (including rapid HIV testing/counseling)

·       Pre-conception counseling (Counseling with a provider about family building and optimizing your health in preparation for a future pregnancy)

·       PrEP (pre-exposure prophylaxis) services

·       Breast/chest and cervical cancer screenings

·       Same day IUD and implant placement appointments may be available

 

Our clinical staff have expertise in the following birth control:

·       Non-hormonal IUD - Paragard (copper) IUD

·       Hormonal IUDs

·       Implant

·       Depo Provera (the shot)

·       Oral contraceptive pills

·       Patch

·       Nuva Ring

·       Diaphragm consultation (only available at PCC South)

·       Plan B/Ella (morning after pill)

·       Condoms

·       Referral and counseling for bilateral tubal ligation (BTL) or vasectomy

·       Outpatient vasectomies (only available at PCC Salud and PCC West Town)

·       Discussion of any other methods available

 

To schedule an appointment for obtaining birth control:

·       If you are already an established patient at PCC, please call your closest PCC clinic to set up an appointment to talk with a provider about your desired birth control method.

·       If you are not currently a PCC patient, please call the closest PCC clinic to set up an in-person, new patient visit. The provider will be able to offer you same-day birth control, but a return visit may be required for IUD or implant placements.

·       For some birth control methods, a telemedicine appointment may be offered instead of an in-person appointment if the patient desires.

 

During your visit, your provider will offer all recommended vaccines. PCC strongly encourages following the Centers for Disease Control and Prevention (CDC) guidelines for all immunizations throughout the lifecycle.

PCC provides primary care sports medicine and musculoskeletal services at the PCC Sports Medicine and Prenatal Wellness Clinic. Patients with musculoskeletal injuries and complaints can be evaluated and managed by Sports Medicine Specialist. Patients may be referred directly from their primary care provider, school athletic trainer, urgent care, or the emergency department.

Services include:

  • Treatment of acute or chronic overuse injuries involving muscle, tendon, ligament, and bone
  • Sport concussion management
  • Pre-participation sports physicals
  • Acute disease management in athletes
  • Coordination of care with physical therapists and occupational therapists
  • Facilitation of transfer of care to orthopedic or sports surgeons, if needed

Books are given to kids aged six months to five years old. Your provider will also help give you tips about reading and helping your child learn to love reading at a young age.

Reach Out and Read is a literacy program of the American Academy of Pediatrics. Reading expands a child’s vocabulary. This can help them do well in school! Reading improves grammar and writing skills. It helps them to be independent and confident.

That is why PCC gives books to kids during appointments. Every child from six months to five years old is given a new book at each well-child check-up. 

We encourage parents and caregivers to read with their children. You can help them love to read!
