Chicago, IL - PCC Salud Family Health Center, part of the PCC Community Wellness Center network of non-profit community health centers, celebrated the grand re-opening of its newly renovated facility on May 13, 2013 with an open house. Luis Arroyo (D), Member of the Illinois House of Representatives, 3rd District, attended and addressed the crowd who assembled to tour the facility. Arroyo voiced a clear support for PCC doctors and nurses who are doing a "heck of a job" and shared his interested in seeing PCC expand further into the neighborhood with more services and clinics.

Oak Park, IL - PCC Community Wellness Center (PCC) is pleased to announce a grant award from the March of Dimes, Illinois Chapter, to support an innovative program aimed to support overweight or obese pregnant women during and after pregnancy. PCC believes that integrated healthcare should extend outside the exam room in order to be most effective. The New Mom’s Wellness Support program will complement the care provided to new moms from providers at the PCC Austin Family Health Center. A Health Support Specialist at PCC will collaborate with staff from the Community and Economic Development Association of Cook County, Women, Infants, and Children Program (CEDA WIC) to deliver weight management and healthy lifestyle programming to new moms.

Chicago, IL - Earlier this year, four Chicago hospitals, six family health centers and one behavioral health provider came together to create MyCare Chicago, an Accountable Care Entity (ACE) and new managed care healthcare plan for individuals and families based in Cook County. MyCare is for children, parents and caregivers who are Medicaid eligible or on Medicaid or All Kids and who currently have an HFS medical card.

Oak Park, IL - In 2011, PCC Community Wellness Center (PCC) committed to providing a new model of care, patient-centered care, at its health centers. Following guidelines set by the National Committee for Quality Assurance (NCQA), five PCC health centers were recognized as Patient-Centered Medical Homes (PCMH) in August 2013. This fall, PCC Austin and Lake Street Family Health Centers further enhanced their services in order to obtain Level 3 recognition by NCQA.

Chicago, IL - Yesterday PCC Community Wellness Center (PCC) held a groundbreaking ceremony for its latest project to improve health outcomes—the PCC Austin Community Farm. Located in Chicago’s Austin neighborhood in an 8,000-square-foot lot across the street from PCC Austin Family Health Center, the farm was made possible by a $350,000 grant from the Humana Foundation. PCC is partnering with the Chicago Botanic Garden, which will provide participants from its urban agricultural program, Windy City Harvest, to install and maintain the farm.

Oak Park, IL - Academy and Emmy award nominated actress Mariel Hemingway is teaming up with PCC Community Wellness Center (PCC) to focus attention on behavioral health issues. Hemingway will be the keynote speaker at PCC’s fifth annual Gala at Oak Brook Hills Resort on October 11. PCC and Hemingway aim to increase awareness of the enormous impact behavioral health issues have on underserved communities.

Berwyn, IL - March 18, 2015 – On Friday, PCC Community Wellness Center (PCC) held a grand opening for the Birth Center at PCC , Illinois’ first free-standing birth center.At the grand opening, U.S. Sen. Dick Durbin (D-IL), and Representative Camille Y. Lilly (D, 78th District) spoke to over 80 attendees about the importance of birth centers and adequate healthcare access.

05/27/15 Fresh from the PCC Austin Community Farm

Oak Park, IL - PCC Austin Produce Market's fifth season begins on Tuesday, June 2 with a new twist: organically grown produce grown on-site by PCC Community Wellness Center (PCC). Instead of simply telling patients to eat well, the community health center is providing easy access to produce within the Austin community. Produce grown on the farm will be harvested and sold at the market. First harvest picks will include mustard and collard greens, kale, radishes, lettuce, and scallions.

Berwyn, IL -  This summer PCC Community Wellness Center will host a series of free community events at the Birth Center at PCC . These “Women’s Night Out” events will foster community, model self-care techniques, and introduce health concepts such as nutrition or physical activity for women and mothers. The first Women’s Night Out event will be held on July 24, from 6:30 p.m. to 8:30 p.m. at 6201 Roosevelt in Berwyn.
