Lisa Kozinski, MD

Lisa Kozinski is a Family Medicine Physician. She received her medical degree from Rush Medical College in 2002 and completed residency at MacNeal Hospital in Berwyn, IL. Dr. Kozinski completed her undergraduate degree in Biology at Northern Illinois University. She speaks fluent Polish.

Chandra Matteson, MSN, APN

Chandra Matteson is a Family Nurse Practitioner at PCC. She received a Master's of Science in Nursing degree from The University of Southern California in 2019, and a Bachelor's in Science in Nursing from Kent State University in 2012. Chandra was a part of the 2020 Cohort of Rush University's OUD Treatment Fellowship's ECHO Program. Before PCC, Chandra worked as an ER nurse for ten years before completing her Master's degree. Prior to that, she worked for Howard Brown Health as a clinical research nurse, worked in emergency medical services, and as an ICU tech. Chandra is learning Spanish and hoping to do an immersion program, preferably someplace with a beach. Outside of work, she enjoys cycling as transportation, petting other people’s dogs, collecting vinyl and music, writing, and cooking.

Yolanda Cardenas, MD, FAIHM

Dr. Cardenas received her medical education from Rush Medical College in 2001. She completed the West Suburban Family Medicine Residency at West Suburban Medical Center. Before medical school, Dr. Cardenas earned a Bachelor of Science, majoring in biology, with a minor in women's studies from Loyola Univeristy, Chicago. She graduated from Loyola's Honor's Program and is a member of the PHI Beta Kappa Society. In 2019, Dr. Cardenas graduated from the Academy of Integrative Health & Medicine Fellowship, with honors. Before PCC, Dr. Cardenas spent six years as locums tenens physician where she practiced medicine in Arizona and California, including assignments at Indian Health Services Clinic. In her free time, she enhjoys music, art, poetry, and writing. Dr. Cardenas is fluent in Spanish.

Leslie Smebak, MD

Dr. Leslie Smebak is from a small town in Minnesota very close to the Canadian border. She grew up with parents who raised cattle. Her introduction to family medicine started there, as it was difficult to find a specialist nearby. Dr. Smebak attended Cornell College in Iowa as an undergraduate. She studied biochemistry there. Prior to attending the University of Chicago – Pritzker School of Medicine in Hyde Park, she worked as a nursing assistant in the University of Iowa hospital system. Dr. Smebak is very committed to the full spectrum practice of family medicine, with a particular interest in obstetrics. She completed residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program (Class of 2021). Someday, she would like to teach medicine to others. Outside of medicine, her interests are focused on the outdoors. She loves hiking, camping, and canoeing.

Morgan Beatty, MD, MPH

Originally from Indianapolis, Indiana, Dr. Morgan Beatty graduated from Columbia University College of Physicians and Surgeons in 2018 and subsequently from the West Suburban/PCC Family Medicine Residency Program in 2021. She then completed a fellowship in Community Health at PCC before joining as a full-time clinician in 2022. In addition to English, she speaks fluent Spanish. Dr. Beatty's interests include reproductive health, behavioral health, and health education. She is also passionate about health policy, health system reform, and community engagement. In her free time, you can probably find her in the garden or enjoying food with friends.

July 30, 2018—Dr. Penny White, pediatrician at PCC Community Wellness Center (PCC) is accepting new patients at two PCC health centers in Chicago’s Austin area. As a pediatric physician, she can care for babies, children, and teens. Services include well child exams, immunizations, treatment for minor or chronic illnesses, lead screening, and school physicals.

Chicago, IL – PCC Community Wellness Center (PCC) recently opened its third health center in West Suburban Medical Center. PCC Community Wellness Center at West Suburban is located inside the hospital’s professional office building at 1 Erie Court, Suite 7140, Oak Park, IL 60302.

The health center is accepting new patients and offers primary care, behavioral health care, and support services. It is open on Mondays, Wednesdays, and Fridays from 8 a.m. until 5 p.m. and on Tuesdays and Thursdays from 8 a.m. until 8 p.m. You can call PCC Community Wellness Center at West Suburban at 773-537-0020 for more information or to schedule an appointment.

Natalie Parker-Renfroe, MD

Specializing in pediatric care, Dr. Parker-Renfroe joined the PCC health care team in 2004. She earned her medical degree from the University of Illinois College of Medicine at Chicago in 1992. Dr. Parker-Renfroe completed residency in pediatrics at Rush University.

Daniel Hawthorne, MD

Dr. Daniel Hawthorne completed his undergraduate education at Wheaton College and his medical education at the University of Illinois College of Medicine in 2014. He completed his residency at PCC, as part of the West Suburban Medical Center and now remains on staff as an attending physician.
