Through developing this new community healthcare and fitness center, PCC aims to create a home supporting its mission, serving as a model in community healthcare. The new building will be a certified WELL Building, a certification program offered by the International WELL Building Institute™ (IWBI™), a roadmap for creating and licensing spaces that promote human health and well-being. IWBI invited PCC to pilot its most impactful program and to become one of the first WELL-certified campuses in the country.

First Floor Clinical Space:

  • Primary Care Clinic: 18 Exam Rooms, 2 Procedure Rooms, 3 Care Team Stations, 1 Lab, 2 Special Services Offices, Registration and Waiting Areas
  • Health and Fitness Center approximately 5,650 sq. ft.

Second Floor Clinical Space:

  • Behavioral Health Suite: 5 Behavioral Health Offices, 2 Group Rooms, a Leadership Office
  • CDC Suite: 5 Exam Rooms, 2 Behavioral Health Offices, 2 Group Rooms, 1 Lab, 2 Counseling Rooms, Dispensing Space
  • Care Coordination: Provider Workspaces, Care Coordination Office
  • Registration, Waiting, and Circulation

Non-Clinical/Staff Areas:

  • First Floor: 3 Offices, Staff Storage
  • Second Floor: Telemedicine Center, Break Room
  • Third Floor: C-Suite Offices, Admin, Assistant Offices, Conference and Meeting Rooms

 


 

The new facility will be located at 5461 West Lake Street, adjacent to its existing Austin Family Health Center in Austin on the south side of Lake Street, between Lotus and Pine. The new building will increase PCC’s capacity to provide health care services to more residents of Austin, given that the existing clinic only has 14 exam rooms and the new clinic will have 24 exam rooms.

Additionally, once primary care services move to the new building, the existing Austin Family Health Center at 5425 West Lake Street will offer on-site specialty care to patients. Presently, PCC patients do not have easy access to specialty care. Soon, they will be able to obtain specialty care right in their neighborhood.

We extend our deepest gratitude to all stakeholders involved in this transformative project, including the PCC Foundation, the City of Chicago, 37th Ward Alderman Emma Mitts, Rep. La Shawn K. Ford, Congressman Danny K. Davis, Cook County Land Bank Authority, among others. Their support and collaboration have been instrumental in bringing this vision to life.

Family Physicians Serve on the Front Lines of Addiction Medicine

While practicing family medicine for many years, I saw how substance use disorders affected many of my patients and their families. To provide them with the best comprehensive care possible, I pursued the addiction medicine fellowship at Rush University Medical Center in Chicago.

It was one of the best career decisions I’ve ever made. Now, during National Addiction Treatment Week, I encourage other family physicians to learn about the critical role we can take in treating patients with SUDs.

Thousands of N-95 masks, new COVID treatment drug gifted to Oak Park health center

CHICAGO — A local health organization has received thousands of masks and another valuable tool in the fight against COVID-19.

In January, the Biden administration started distributing millions of masks to pharmacies and health centers across the country. 

Late last week, the PCC Community Wellness Center, with locations across Chicagoland, received thousands of masks and a Covid-fighting drug not yet well known.
