Medicaid Redetermination & Enrollment
PCC Community Wellness Center is hosting a Medicaid Redetermination & Enrollment Event to help community members maintain their health coverage and access essential benefits. During this event, participants can receive 1:1 support with Medicaid redetermination and SNAP enrollment, learn about CountyCare Medicaid Visa Rewards, and receive a free blood pressure screening.
The event is free and open to the public, and the first 20 attendees will receive free produce provided by Foodsmart. Community members are encouraged to bring documentation such as proof of income from the last 30 days, identification or proof of citizenship, and their Medicaid redetermination letter to receive assistance.
This event is hosted in collaboration with Chicago CARES and the Greater Chicago Food Depository, working together to support community members in maintaining access to healthcare and food resources.
