Jerrard Walker, RN, LCSW, CADC, SAP

Chief Population Health Officer, Jerrard Walker, RN, LCSW, CADC, SAP, CAMS-II joined PCC in 2020. He obtained his Master of Social Work from Wayne State University - School of Social Work. He also has a Bachelor of Science in Nursing from Resurrection University and a Bachelor of Arts in Sociology, with a Minor in Social Work from Oakland University. Jerrard completed his social work training at Henry Ford Hospital in Detroit, MI. He was one of the architects who established the CCHHS CountyCare program. In his free time, he enjoys playing tennis, traveling, and reading.

Katrina Weimer, MD

Dr. Weimer obtained her medical degree at the University of Toledo College of Medicine in Toledo, Ohio. She completed the University of Wisconsin's Family Medicine Residency Program in Madison, Wisconsin. Dr. Weimer also completed PCC's 2020-2021 PCH Fellowship program. She is dedicated to obstetrics/perinatal health and addiction medicine. Outside of medicine, her interests include kayaking, volleyball, rock climbing, hiking, biking, spending time with her family, thrifting, and home improvement projects.

Bianca Cervantes, LPC

Bianca Cervantes is a Licensed Professional Counselor at PCC. She received her Master's of Arts in Clinical Psychology from Roosevelt University in 2017. She completed her undergraduate degree at Chicago State University, majoring in Psychology. Bianca completed an internship at the Pediatric Developmental Center at Illinois Masonic Medical Center. She formed part of the Latino Mental Health Program at the center that provided individual therapy to children with Autism and other psychological disorders. Bianca co-facilitated parenting psychoeducation therapy groups and has experience working in a private practice providing therapy to a diverse population of patients. Her professional interests include autism, anxiety, depression, and ADHD. Outside of work, she enjoys spending time reading, traveling, and spending time with family.

Kathryn Nutting, MD

Dr. Kathryn Nutting is completing residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. She received her medical education from The University of Chicago Pritzker School of Medicine. Dr. Nutting also studied public health and biochemistry as an undergraduate at the University of Massachusetts Amherst. In between her undergraduate and medical studies, she worked as a nursing assistant. In her free time, she enjoys hanging out with friends, cooking, and outdoor activities such as swimming, biking, and hiking.

Jacqueline Greene-Roper, MD

Dr. Jacqueline Greene-Roper is completing residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. She received her medical education from Loyola University Chicago Stritch School of Medicine. Dr. Greene-Roper also received an undergraduate degree in Justice & Society from Creighton University in Omaha, Nebraska. Between her undergraduate studies and medical school, Dr. Greene-Roper lived in Peru for two years as a volunteer in a hospice center. Prior to medical school, she also worked in home health and as a nanny. In her free time, she loves spending time at her family cabin in Wisconsin, being outdoors, and swimming.

Megan Dawson, CNM

Megan Dawson is a Certified Nurse Midwife. She completed her RN and Midwifery degree at the University of Illinois at Chicago, College of Nursing. Before nursing, she received a Bachelor's in Spanish from Northern Illinois University and a Master's in Business Administration from the University of Mexico. Megan completed the 2014-2015 Albert Schweitzer Fellowship and had a previous career in political fundraising in Washington D.C. and New Mexico. In her free time, Megan likes to play board games, cook, go on beach days, hike, and play "mermaid" with her 4-year-old. She speaks Spanish.

Sara Hogue, MSW

Chief Performance Improvement Officer, Sara Hogue, joined PCC in 2015. Sara is passionate about public health and committed to developing quality assurance tools that can improve population health. She has a decade of experience in performance improvement in such areas as research, program evaluation, data analysis, quality assurance, and performance management. As PCC’s CPIO, Sara leads initiatives to develop new capacity in quality improvement across the organization. Prior to her promotion to CPIO, Sara served as PCC’s Director of Performance Improvement, where she managed the National Committee for Quality Assurance’s Patient Centered Medical Home recognition process, expanding PCC’s recognition from four to eight sites of care. Sara also pioneered the organization’s Core Culture Committee, which works to identify and address inclusivity needs for patients and staff. Sara holds a Master of Social Work from DePaul University and a Bachelor of Arts from St. Xavier University.

Jeanine Valrie-Logan, MPH, CNM

Jeanine Valrie-Logan is a Certified Nurse Midwife. She received a Bachelor of Arts from Fisk University and a Master in Science in Nursing from DePaul University. She also received a Master of Public Health with a concentration in Global Reproductive Health from the George Washington University. Jeanine completed the nurse midwifery program at Frontier Nursing University in the fall of 2018. She loves being outside with her family and is interested in herbs, plants, and growing food. Jeanine promotes breastfeeding and has co-authored Free to Breastfeed: Voices of Black Mothers - a book that highlights the experiences of Black women who breastfeed.

Yolanda Cardenas, MD, FAIHM

Dr. Cardenas received her medical education from Rush Medical College in 2001. She completed the West Suburban Family Medicine Residency at West Suburban Medical Center. Before medical school, Dr. Cardenas earned a Bachelor of Science, majoring in biology, with a minor in women's studies from Loyola Univeristy, Chicago. She graduated from Loyola's Honor's Program and is a member of the PHI Beta Kappa Society. In 2019, Dr. Cardenas graduated from the Academy of Integrative Health & Medicine Fellowship, with honors. Before PCC, Dr. Cardenas spent six years as locums tenens physician where she practiced medicine in Arizona and California, including assignments at Indian Health Services Clinic. In her free time, she enhjoys music, art, poetry, and writing. Dr. Cardenas is fluent in Spanish.

Jacob Smith, MD

Dr. Smith received his medical education from Rush University in 2016. He completed the Northwestern McGaw Family Medicine Residency in Humboldt Park. Prior to medical school, he completed two years of AmeriCorps service at PCC Salud Family Health Center. Dr. Smith's hobbies include gardening, cooking, playing basketball, and spending time and traveling with his wife, daughter, and two cats. He speaks fluent Spanish.
