PCC is excited to celebrate its 25th anniversary! Over the years, PCC has expanded to eleven sites. As a Federally Qualified Health Center, PCC is committed to providing quality, affordable, and accessible primary health care and support services for medically underserved communities on the West and Northwest Sides of Chicago and in the near west suburbs. In 2016, PCC served 48,357 individual patients.

In addition to primary care, PCC offers dental health care in state-of-the-art facilities and integrated behavioral health services. PCC is proud to have launched Illinois' first freestanding birth center, built a community farm, and established an integrated Chemical Dependency Program to expand substance abuse treatment. The need for high quality, affordable health care is immense; therefore, PCC is seeking additional support from the community to sustain and expand this critical cause.

PCC Foundation anticipates support for the gala with attendance of up to 250 physicians, health care providers, and prominent civic and business leaders, in addition to PCC staff and local supporters from Chicago and the Oak Park-River Forest area.
