What we offer our community

Our Health Care Services Include:
• Pediatric and Adolescent health care that includes well-child exams, immunizations, sick infant/child exams, school and sports physicals, and lead screenings
• Women’s Health Care (ob/gyn) that includes maternity care and delivery, Pap smears, and breast cancer screenings
• Adult Health Care
• Senior Health Care
• Diagnosis and treatment of illness
• Disease screening and prevention
• Family planning (birth control)
• Management of chronic illnesses such as asthma, diabetes, and high blood pressure
• HIV testing, counseling, and primary care
• Dental care and referral
 
Note: The Oak Park Health Department now offers confidential testing and treatment for sexually transmitted disease (STD) as well as anonymous or confidential testing for HIV. Appointments are necessary. Testing and treatment is free for Oak Park residents, but non-residents are charged a fee based on a sliding income-based scale. To schedule a confidential appointment, call the Oak Park Health Department at (708) 358-5483.

• WIC form completion

In order to ensure our services are affordable and accessible:
• We accept many major health plans
• We accept Medicaid and Medicare
• We offer a sliding fee scale based on total family income and size to individuals who lack health insurance
• We provide application assistance for Illinois KidCare and FamilyCare
• Spanish speaking medical and support staff are available on-site

PCC Community Wellness Center, in partnership with its physicians and staff, provides quality medical services to the medically underserved in the community.
