Medical Providers
  • Katrina Cordero, MD – Family Medicine Physician (Female)

  • Dr. Cordero is a family medicine physician who joined PCC in 2013. She serves as the Medical Director at PCC Walk-In Wellness Center at West Suburban Medical Center. Dr. Cordero obtained her medical degree from the University of Chicago – Pritzker School of Medicine in 2002. She completed residency at West Suburban Medical Center Family Residency Program, working specifically at PCC Salud Family Health Center. A true change maker, Dr. Cordero served as a Peace Corps volunteer in the Dominican Republic. In her leisure time, she enjoys baking, reading, and studying other languages. Dr. Cordero is fluent in Spanish and is the proud mother of six children.



  • Natasha Diaz, MD – Family Medicine Physician (Female)

  • Having joined PCC in 2007, Dr. Diaz specializes in family medicine. She received her medical education at the University of Illinois College of Medicine at Chicago in 2001. Dr. Diaz completed residency at Contra Costa Regional Medical Center in California. She is fluent in both English and Spanish.



  • Patrick Ennis, MD – Family Medicine Physician (Male)

  • Dr. Ennis joined PCC in 2016 and specialized in family medicine. He completed his medical education at New York Medical College and finished residency training at West Suburban Medical Center Family Residency Program. Dr. Ennis speaks English and Spanish. In his leisure time, he enjoys bicycling, spending time with his dog, alpine skiing, and travelling.



  • Jill Hawkes, APN – Family Nurse Practitioner (Female)

  • Jill Hawkes is an advanced nurse practitioner who joined PCC in 2013. She completed her nursing education at the University of Illinois at Chicago – College of Nursing in 1996. Outside of nursing, she enjoys gardening, reading, and bowling.



  • Daniel Hawthorne, MD – Family Medicine Physician, Resident (Male)

  • Dr. Hawthorne completed his undergraduate education at Wheaton College and his medical education at the University of Illinois College of Medicine in 2014. He is completing residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. Dr. Hawthorne is passionate about the cultural concepts of health and wellness and the relationship between diet and a person's perception of chronic disease. He is committed to bridging the cultural gap between doctors and their patients to communicate more effectively and provide better patient care. Dr. Hawthorne grew up in Bolivia, so he is fluent in Spanish and loves watching and playing soccer. He also enjoys playing and listening to music, running with his dog, and going on adventures, including exploring Chicago’s amazing restaurants.



  • Whitney Henderson, APN – Family Nurse Practitioner (Female)

  • Whitney Henderson is a board-certified family nurse practitioner who received her nursing education at Loyola University Chicago in 2011. She is passionate about nursing and particularly interested in contraceptive management. Outside of nursing, Whitney enjoys spending time with her family and reading.



  • Regina Kim, MD – Family Medicine Physician (Female)

  • Specializing in family medicine, Dr. Kim earned her medical degree at the University of Chicago – Pritzker School of Medicine in 2002. She completed residency at West Suburban Medical Center. Dr. Kim joined PCC in 2013 and speaks English, Korean, and Spanish.



  • Jenna Pederson, APN – Family Nurse Practitioner (Female)

  • Jenna Pederson is an advanced nurse practitioner who completed her nursing education at Rush University in 2016. Prior to joining PCC in 2016, Pederson served as an emergency room nurse. Her other interests include running, baking, and traveling.



  • Sabina Wong, MD – Family Medicine Physician (Female)

  • Dr. Wong is a family medicine physician who joined PCC in 2015. After completing her undergraduate education at Stanford University, she earned her medical degree from Vanderbilt University School of Medicine, which honored her with the Tennessee Academy of Family Physicians Award upon graduation in 2006. She completed residency at West Suburban Family Medicine Residency Program. Dr. Wong is passionate about using patient education for healthy lifestyle promotion, reduction of obesity, community development, provider wellbeing, and professional mentorship. In her leisure time, she enjoys giggling with her three little girls, hiking, and creating art projects.



Behavioral Health Providers
  • Haley Swanson, LCSW – Behavioral Health Consultant (Female)

  • Haley Swanson joined PCC’s behavioral health care team in 2016. She is a Licensed Clinical Social Worker and Certified Alcohol Drug Counselor. Haley earned her Master of Social Work from the University of California in Los Angeles in 2010. Prior to focusing on substance use treatment at PCC, she worked for several years in the community mental health field providing services to adults with severe and persistent mental illness. Passionate about mental wellness, Haley is trained in providing trauma therapy to women, dual diagnosis treatment, and domestic violence. Outside of social work, she enjoys spending time with her children and dog.
