Medical Providers
  • Cecelia Bacom, CNM – Certified-Nurse Midwife (Female)

  • Ceal Bacom is a certified nurse-midwife who joined PCC in 2004. She completed her Bachelor of Arts in Spanish Literature at the University of Virginia. She later obtained her Bachelor of Science in Nursing at Northwestern University and a Master of Science in Nursing at the University of Illinois at Chicago in 1988. She completed clinical rotations primarily at Cook County Hospital (now Stroger Hospital). She has extensive experience providing care for families both in and out of the hospital setting. Ceal has received numerous honors including an Award for Innovative Midwifery Care from the American College of Nurse-Midwives – Illinois Affiliate and the Joyce Roberts Award for Excellence in Midwifery. She is fluent in Spanish. In her leisure time, Ceal enjoys gardening, reading, and bike riding.



  • Raymond Baxter, MD– Family Medicine Physician, Fellow (Male)


  • Anastasia Crihfield, MD – Family Medicine Physician, Resident (Female)

  • Dr. Crihfield completed both her undergraduate and medical education at the University of Virginia (UVA). She is completing her residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. Dr. Crihfield is a member of the Alpha Omega Alpha Honor Society and the Gold Humanism Honor Society and was honored as a Generalist Scholar while attending UVA. Outside of medicine, she enjoys knitting, yoga, houseplants, visiting national parks, and spending time at Chicago beaches.



  • Julia Eckersley, MD – Family Medicine Physician (Female)

  • Dr. Eckersley joined PCC in 2004 and specializes in family medicine. She received her medical degree at Rush University – Rush Medical College in 2001 and completed residency at West Suburban Medical Center Family Residency Program. Dr. Eckersley is a member of the Alpha Omega Alpha Medical Honor Society. She is fluent in Spanish.



  • Tiffany Forte, MD – Family Medicine Physician, Resident (Female)

  • Dr. Forte earned her medical degree from Loyola University Chicago – Stritch School of Medicine in 2016. At Loyola, she was accepted into the Global Health Honors Program. She is completing residency in family medicine here at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. Her medical interests include women’s health, dermatology, prevention, in-office procedures, and caring for medically underserved populations. Dr. Forte is fluent in Spanish.



  • Jeanne Gambucci, DO – Family Medicine Physician, Resident (Female)

  • Dr. Gambucci is completing her residency at PCC as part of the West Suburban Medical Center – Family Medicine Residency Program. She earned her medical degree from Touro University California in 2014. Before attending medical school, she worked as a tuberculosis clinical trials coordinator in Barcelona, Spain for three years. Outside of medicine, she enjoys spending time with her family, participating in sports, and traveling.



  • Katherine Herold, CNM – Certified-Nurse Midwife (Female)

  • Katherine Herold is a certified nurse-midwife who completed her midwifery training at the University of Illinois at Chicago – College of Nursing in 2011. Katherine joined PCC in 2012. She speaks Spanish fluently. Her other interests include bicycling, writing, improvisational comedy, and traveling.



  • Jennifer Jaume, CNM – Certified Nurse-Midwife (Female)

  • Jen Jaume received her Master of Science in Nurse-Midwifery from the University of Illinois at Chicago in 2014. She boasts a strong background in caring for mothers and families, having worked as a doula and homebirth nurse. Born in Cuban Miami, Jen came to Chicago to raise her family. The many things she loves include dancing, laughing, reading, cooking, and mothering. Jen is fluent in Spanish and English.



  • Marjorie Kozlowksi, APN – Family Nurse Practitioner (Female)

  • Marjorie Kozlowski is an advanced nurse practitioner who joined PCC in 2015. She obtained a Master of Science in Nursing from Northern Illinois University in 2003. Marjorie is the recipient of the Helene Fuld Scholarship. Outside of nursing, she loves painting on canvas. She is currently a Clinical Assistant Professor at DePaul University and a doctoral student at Vanderbilt University.



  • Michael Ly, MD – Family Medicine Physician (Male)

  • Dr. Ly is a family medicine physician who joined PCC in 2014. He completed both his undergraduate and medical education at Loyola University Chicago – Stritch School of Medicine in 2011. Dr. Ly completed residency at West Suburban Medical Center – Family Medicine Residency Program in 2014. He speaks English, Spanish, and Khmer.



  • Katherine Lynch, MD – Family Medicine Physician (Female)


  • Tera Martin, CNM – Certified-Nurse Midwife (Female)

  • Tera Martin graduated from the University of Illinois at Chicago in 2009 with a Master of Science in Nurse-Midwifery. Since then, she has worked as a homebirth midwife in Chicago and the nearby suburbs. She is committed to providing holistic, evidence-based midwifery care to women who birth outside of hospitals. In her free time, Tera enjoys traveling and hiking. She once combined her love of both during a 500 mile hike through Spain.



  • Kristen Moser, MD – Family Medicine Physician, Resident (Female)

  • Dr. Moser earned her medical degree from Loyola University Chicago – Stritch School of Medicine in 2016. She is completing residency here at PCC, as part of the West Suburban Medical Center Family Residency Program. Before changing course and entering medical school, she taught high school math in Guatemala for two years. Outside of medicine, Dr. Moser enjoys traveling, reading, running, and cooking.



  • Alexander Neuman, MD – Family Medicine Physician, Resident (Male)

  • Dr. Neuman earned his medical degree at Chicago College of Osteopathic Medicine at Midwestern University in 2015. He is completing residency at PCC as part of the West Suburban Family Medicine Program. He has been honored with membership in the Gold Humanism Honor Society. He has also conducted sleep research. Dr. Neuman is fluent in Spanish. Outside of medicine, he enjoys biking, playing the piano, and enjoying meals with family and friends. He and his wife are the proud parents of a little boy.



  • Annette Payot, CNM – Certified-Nurse Midwife (Female)

  • Annette Payot provides leadership to PCC’s Midwives as the Director of Midwifery. She is a Certified Nurse-Midwife and Women’s Health Nurse Practitioner who joined PCC in 2009. Annette completed her nursing and midwifery education at the University of Pennsylvania and the University of Illinois at Chicago in 2007. With an undergraduate degree in international studies, she spent many years working with international grants programs and volunteered with the Peace Corps in Mongolia. Annette loves all things related to natural childbirth. In addition to midwifery, she enjoys running, jogging, and listening to music. Annette speaks both English and Spanish.



  • Elizabeth Redican, DO – Family Medicine Physician, Fellow (Female)


  • Lizbeth Rodriguez, MD – Family Medicine Physician, Resident (Female)

  • Dr. Rodriguez is a family medicine physician who obtained her medical degree from the University of Illinois College of Medicine at Chicago in 2014. Completing residency through the West Suburban Medical Center – Family Medicine Residency Program, Dr. Rodriguez is a third year resident at PCC. She speaks Spanish fluently.

  • Gitanjali Sivasubramaniam, MD – Family Medicine Physician (Female)


  • Moshini Sivasubramaniam, MD – Family Medicine Physician, Resident (Female)

  • Dr. Sivasubramaniam is completing her residency at PCC as part of the West Suburban Family Medicine Program. After graduating from Northwestern University, she earned her medical degree from Loyola University Chicago – Stritch School of Medicine. Dr. Sivasubramaniam is fluent in both English and Spanish. In her free time, she enjoys traveling, dancing, painting, and cooking.



  • Fernando Velasquez, APN – Family Nurse Practitioner (Male)

  • Fernando Velasquez is a board-certified advanced nurse practitioner who joined the PCC health care team in 2016. He completed both his undergraduate and nursing education at the University of Illinois at Chicago. He has worked as a Registered Nurse in multiple settings including medical detoxification, psychiatry, endocrinology, oncology, and primary care. Fernando is fluent in both English and Spanish.



  • Katherine Wehrmann, CNM – Certified Nurse-Midwife (Female)

  • Katy Wehrmann is a certified-nurse midwife who joined PCC in 2013. After completing her Bachelor of Science in Biology at North Park University, she completed her Master of Science in Midwifery at the University of Illinois at Chicago College of Nursing in 2013, graduating magna cum laude. Prior to nursing school, Katy served as an AmeriCorps volunteer at PCC where she worked as a doula and maternal and child health educator. In her leisure time, she enjoys running, biking, reading, and occasionally cooking elaborate meals.



  • Amy Willen, CNM – Certified-Nurse Midwife (Female)

  • Amy Willen received her Master of Science in Nursing from Vanderbilt University in 2004 and has since attended births as a nurse-midwife in several settings including both large and smaller hospitals, free standing birth centers, and homes in Chicago and surrounding suburbs. Amy believes in natural childbirth and loves being able to tailor her midwifery care to support women's choices no matter what setting they are in. Amy enjoys traveling and has lived in Thailand and worked as a midwife in Uganda and Tanzania. She loves learning new languages but speaks mostly English and Spanish. Committed to providing care in underserved areas, Amy is a member of the National Health Service Corps.



  • Alexander Wu, MD – Family Medicine Physician (Male)

  • Dr. Wu is a family medicine physician who joined the PCC health care team in 1997. Dr. Wu is PCC’s Maternal Child Health Fellowship Co-Director. He earned his medical degree from the University of California Los Angeles in 1992. He completed residency at Long Beach Memorial Family Medicine. Dr. Wu has been honored with four family medicine teaching awards since 1995 and currently teaches at Loyola University Chicago and Northwestern University. In addition to medicine, he enjoys fragrance and flavor science and Latin music and dance. He has even worked in the food and beverage industry and musical theatre. He speaks both English and Spanish.




    Behavioral Health Providers
    • Ricardo Aguinaga, Jr., LCSW – Behavioral Health Consultant(Male)

    • Ricardo Aguinaga, Jr. joined the PCC behavioral health care team in 2016. He is a Licensed Clinical Social Worker who attained his Master of Social Work from the University of Chicago – School of Social Service Administration in 2010. Ricardo completed internships at Pillars and Perspectives Charter School – Joslin Campus. He was honored with the Perspectives Charter School Network Award. Prior to his career in behavioral health, Ricardo developed and ran a youth center in Cicero. He speaks both English and Spanish. In his spare time, Ricardo enjoys playing basketball, hanging out with his dog Cholo, watching movies, and attending Chicago Bears games.



    • Heather Morales, LCSW – Behavioral Health Consultant (Female)

    • Heather Morales is a Licensed Clinical Social Worker who earned her Master of Social Work from the University of Chicago – School of Social Service Administration in 2009. As the Clinical Support Services Manager, Heather provides leadership to clinical care managers, who strategically assist high-risk patients. She speaks both English and Spanish. In her spare time, Heather enjoys yoga, dance, music, gardening, and spending time in nature.



    • Gina Rogers, LCSW – Behavioral Health Consultant (Female)

    • Gina Rogers is a Licensed Clinical Social Worker who attained her Master of Social Work from the Jane Addams College of Social Work at the University of Illinois at Chicago in 2015. Prior to joining PCC, Gina managed youth programs and development projects in the West Humboldt Park and Back of the Yards communities. She also provided screening and advocacy services as an educational rehabilitation consultant. As a bilingual behavioral health consultant, Gina competently cares for both English and Spanish speaking patients.
