Medical Providers
  • Kyle Bohan, DO – Family Medicine Physician, Resident (Male)

  • Dr. Bohan earned his medical degree from A.T. Still University – Kirksville College of Osteopathic Medicine in 2015. He is completing his residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. He also holds a Master in Public Health from the University of Arizona. In his leisure time, he enjoys hiking, traveling, exploring restaurants, and watching and participating in sports.



  • Andrew Eastman, MD – Family Medicine Physician, Fellow (Male)

  • Dr. Eastman earned his medical degree at Jefferson Medical College of Thomas Jefferson University in Philadelphia in 2013. He completed residency at Georgetown University Medical Center and Providence Hospital Family Medicine, both in Washington, DC. Dr. Eastman joined PCC in 2016.



  • Kelly Ellis, CNM – Certified Nurse-Midwife (Female)

  • Kelly Ellis is a certified nurse-midwife who joined the PCC health care team in 2013. She completed her nursing education at the University of Cincinnati in 2010. Kelly is also an International Board Certified Lactation Consultant (IBCLC) and can offer extensive breastfeeding support for patients. Outside of midwifery, Kelly enjoys photography, running, and traveling.



  • Krzysztof Garbarz, DNP – Family Nurse Practitioner (Male)

  • Dr. Garbarz earned his Doctor of Nursing Practice degree from Rush University in 2016, and joined PCC shortly thereafter. He speaks English, Polish, and Spanish. In his leisure time, Dr. Garbarz enjoys hiking and traveling.



  • Kyle Geissler, MD – Family Medicine Physician (Female)

  • Dr. Geissler is a family medicine physician who received her medical training at the Brody School of Medicine at East Carolina University and completed residency at West Suburban Medical Center. She secured a fellowship in sports medicine from Ohio State University. As a result, Dr. Geissler serves at PCC’s Sports Medicine Clinic, located at the Walk-In Wellness Center at West Suburban Medical Center. Prior to joining PCC full-time in 2013, she served at PCC as an AmeriCorps volunteer in 2001. She has been honored with the NCAFP Blair Leadership Award and the Brody Medical School Class Appreciation Award. With a passion for wellness, Dr. Geissler has also worked for the Center for Mind-Body Medicine in Washington, D.C., where she helped whole populations heal from psychological trauma and stress. Outside of medicine, Dr. Geissler enjoys swimming and participating in triathlons.



  • Jennifer Jaume, CNM – Certified Nurse-Midwife (Female)

  • Jen Jaume received her Master of Science in Nurse-Midwifery from the University of Illinois at Chicago in 2014. She boasts a strong background in caring for mothers and families, having worked as a doula and homebirth nurse. Born in Cuban Miami, Jen came to Chicago to raise her family. The many things she loves include dancing, laughing, reading, cooking, and mothering. Jen is fluent in Spanish and English.



  • Jessica Johnson, MD, MPH – Family Medicine Physician, Fellow (Female)

  • Dr. Johnson is a family medicine physician who received her medical degree from the University of Connecticut in 2012. Immediately after completing her residency at Oregon Health and Science University, she joined PCC as a Fellow in 2016. She also holds a Master’s in Public Health from the University of Connecticut.



  • David Kim, MD – Family Medicine Physician (Male)

  • Dr. Kim is a family medicine physician who joined PCC in 2016. After completing his undergraduate studies at Northwestern University, he received his medical degree at the University of Illinois College of Medicine at Chicago in 2003. Dr. Kim completed residency at West Suburban Medical Center.



  • Jenny Kim, MD – Family Medicine Physician (Female)


  • Madalyn Kovarik, MD – Family Medicine Physician, Resident (Female)

  • Dr. Kovarik is a family medicine physician who earned her medical degree from Loyola University Chicago – Stritch School of Medicine in 2016. At Loyola, she was accepted into the Global Health Honors Program. She is completing residency in family medicine here at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. In addition to medicine, Dr. Kovarik enjoys wine making, cooking, skiing, and traveling.



  • Emily Lovaasen, MD – Family Medicine Physician (Female)

  • Dr. Lovaasen is a family medicine physician who joined PCC in 2011. She earned her medical degree from Loyola University Chicago – Stritch School of Medicine in 2008 and completed residency at West Suburban Family Medicine Residency.



  • Gina Marcin, CNM – Certified Nurse-Midwife (Female)

  • Gina Marcin is a certified nurse-midwife who completed her nursing education at the University of Illinois at Chicago in 2013. With a background as an herbalist and acupuncturist, she treats mothers and families from a holistic perspective. Gina is dedicated to the midwifery model of care and is eager to support mothers in the out-of-hospital birth setting. For recreation, Gina enjoys camping, gardening, and fire-tending. Each year, she also co-organizes a medicinal herbal conference, The Midwest Herb Fest, in southern Indiana.



  • Patricia Martin, DO – Family Medicine Physician, Resident (Female)

  • Dr. Martin is completing her residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. She received her medical degree from Rowan University – School of Osteopathic Medicine in 2014. She is a National Health Service Corps Scholar. Dr. Martin also speaks Spanish. In her leisure time, she enjoys running, reading, cooking, traveling, and spending time with friends and family.



  • Kathleen McDonough, MD – Family Medicine Physician (Female)

  • Dr. McDonough joined the PCC health care team in 2009. She earned her medical degree from Loyola University Chicago – Stritch School of Medicine in 2006. She completed residency at West Suburban Medical Center – Family Medicine Residency Program. To enhance her capacity to care for patients, Dr. McDonough has obtained licensing to provide Suboxone to patients addicted to opiates. In her free time, she enjoys traveling and spending time with her family.



  • Michele Medley, DO – Family Medicine Physician, Resident (Female)


  • Maureen Milota, APN – Family Nurse Practitioner (Female)

  • Maureen Milota is an advanced nurse practitioner who joined PCC in 2011. She received her nursing education at Rush University in 2010. Maureen completed clinical training at the University of Arizona Center for Integrative Medicine. She is a U.S. Department of Health & Human Services National Health Service Corps Scholar and a member of the Sigma Theta Tau International Nursing Honor Society. She is currently an adjunct faculty member at Rush University College of Nursing and is trained as an Asthma Educator and Certified Diabetic Educator. Prior to nursing, Maureen worked as a firefighter and paramedic. In her leisure time, she enjoys gardening, dog walking, and traveling.



  • Sara Miranda, APN – Family Nurse Practitioner (Female)

  • Sara Miranda is a family nurse practitioner who joined PCC in 2014. She completed both her undergraduate and medical education at Resurrection University in 2014. She completed residency at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. Prior to medical school, Sara worked for six years as a registered nurse in the Intensive Care Unit. She has been specially trained to treat Hepatitis C and licensed to provide Suboxone to patients addicted to opiates. Outside of work, Sara enjoys decorating and interior design.



  • Steven Smrha, DO – Family Medicine Physician (Male)

  • Dr. Smrha is a family medicine physician who received his medical degree from Chicago College of Osteopathic Medicine at Midwestern University. Dr. Smrha completed residency at West Suburban Medical Center Family Residency Program. He joined the PCC health care team in 2015. To enhance his capacity to care for patients, Dr. Smrha has obtained licensing to provide Suboxone to patients addicted to opiates.



  • Christine Swartz, MD – Family Medicine Physician (Female)

  • Dr. Swartz is a family medicine physician who joined the PCC health care team in 2008. She earned her medical degree from the University of Vermont College in 2005 and completed residency at the Medicine Central Maine Medical Center. Dr. Swartz serves as PCC’s Medical Director of Electronic Health Records.



  • Sarah Tarnowsky, DO – Family Medicine Physician, Resident (Female)

  • Dr. Tarnowsky received her medical training at Chicago College of Osteopathic Medicine at Midwestern University in 2016. She is completing her residency in family medicine at PCC, as part of the West Suburban Medical Center – Family Medicine Residency Program. In her leisure time, she enjoys traveling, food, wine, hiking, boating, camping, snowboarding, listening to music, and yoga. Before joining PCC, she worked as a yoga teacher.



  • Alisha Thomas, MD – Family Medicine Physician (Female)

  • With a specialization in family medicine, Dr. Thomas has served at PCC since 2008. She received both her undergraduate and medical education at Northwestern University in 2005. Dr. Thomas completed residency through West Suburban Medical Center – Family Medicine Residency Program. As a skilled practitioner, Dr. Thomas has been recognized by the American Academy of Family Physicians as the recipient of the 2015 Exemplary Teaching Award. She was also honored by the Illinois Academy of Family Physicians as the recipient of the 2014 Family Medicine Teacher of the Year. In her leisure time, Dr. Thomas enjoys reading, completing jigsaw puzzles, and staying active with her local church.



  • Brooke Turnock, MD – Pediatrician (Female)

  • As a pediatrician, Dr. Turnock provides compassionate care to infants, children, and adolescents. She serves as the Medical Director at PCC Austin Family Health Center. Dr. Turnock earned her medical degree from Michigan State University College of Human Medicine in 2009 and completed residency at Rush Medical Center. She is a member of the Gold Humanism Honor Society. Prior to joining PCC in 2009, Dr. Turnock gained experience in the mental health field, working with adults with dual diagnoses. She enjoys spending time with her family, reading, dancing, and thrift store shopping.



  • Errin Varnado, DO – Family Medicine Physician, Resident (Female)

Behavioral Health Providers
  • Kelli Bosak, LSW, CADC – Behavioral Health Consultant (Female)

  • Kelli Bosak is a Licensed Social Worker and Certified Alcohol Drug Counselor who joined the PCC behavioral health care team in 2015. She attained her Master in Social Work from the University of Chicago – School of Social Service Administration in 2015. In 2014, Kelli was chosen as an Albert Schweitzer Fellow to address health disparities and the social determinants of health among Chicago’s low-income population. She speaks both English and French. In her leisure time, Kelli enjoys cooking, walking her dog, gardening, yoga, and meditation. She is so passionate about yoga that she was instrumental in PCC’s collaboration with YogaCare to bring yoga services to PCC patients.



  • Amanda Brooks, LCSW, CADC – Behavioral Health Consultant (Female)

  • Amanda Brooks, Director of Behavioral Health at PCC, is a Licensed Clinical Social Worker and Certified Alcohol Drug Counselor. She earned her Master in Social Work from the University of Chicago – School of Social Service Administration in 2012. She also serves as the facilitator of the quarterly behavioral health round table of behavioral health directors affiliated with the Illinois Primary Health Care Association. Prior to her career in behavioral health, she worked as a Radio City Rockette for five years. In her leisure time, Amanda enjoys reading, exercising, and spending time with family.



  • Jill Degen, APN – Psychiatric Nurse Practitioner (Female)

  • Jill Degen is a board-certified psychiatric-mental health nurse practitioner who completed both her undergraduate and graduate education at the University of Illinois at Chicago in 2010. Jill joined the PCC behavioral health care team in 2011.



  • Mathew Lindsay, LSW – Behavioral Health Consultant (Male)

  • Mathew Lindsay is a Licensed Social Worker who earned his Master of Social Work from the Jane Addams College of Social Work at the University of Illinois at Chicago in 2016. Prior to joining PCC, he served in the U.S. Air Force and worked as a teacher for Chicago Public Schools. In his leisure time, Mathew enjoys cycling.



  • Molly Reynolds, LCSW, CCTP – Behavioral Health Consultant (Female)

  • Molly Reynolds is a Licensed Clinical Social Worker and Certified Clinical Trauma Professional focused on providing individual, couple, and family counseling services. She obtained her Master in Social Work from the University of Chicago in 2013. Molly completed a clinical internship in adolescent mental health Partial Hospitalization Program (PHP) at Northshore University health system, and another internship at PCC Community Wellness Center. Before returning to PCC to work full time in 2015, Molly worked for two years in an adolescent mental health PHP and an Intensive Outpatient Program (IOP).


Dental Providers
  • Alaba Adelakun, DDS – Dentist (Male)

  • Dr. Adelakun earned his dental degree from the University of Illinois at Chicago – College of Dentistry in 2009. He completed residency at Shawnee Health Services. Dr. Adelakun is experienced in general dentistry, including surgical and simple extractions and endodontic and periodontal therapies.



  • Justyna Klatka, RDH – Dental Hygienist (Female)


  • William Madison, DDS – Dentist (Male)

  • Dr. Madison received his dental degree from the University of Illinois at Chicago – College of Dentistry in 2011. He completed residency at Advocate Illinois Masonic Medical Center. Dr. Madison is well-versed in general dentistry, including oral surgery and endodontics. He also has extensive training in providing special patient care for patients with developmental disabilities. Dr. Madison joined the PCC dentistry team in 2012.



  • Ryan Tuscher, DDS – Dentist (Female)

  • Dr. Tuscher earned her dental degree from the University of Illinois at Chicago – College of Dentistry in 2009. She completed residency at Advocate Illinois Masonic Medical Center. Dr. Tuscher serves as PCC’s Dental Director. She is highly experienced in performing preventative, restorative, and emergent dental care for adults and children. Dr. Tuscher is passionate about dentistry and has volunteered her time frequently to provide comprehensive dental treatment to underserved patient populations.
