About PCC Community Wellness Center

 


Mission

The mission of PCC Community Wellness Center is to improve health outcomes for the medically underserved community through the provision of high quality, affordable, and accessible primary health care and support services. We are committed to serving the needs of men, women, and children in all lifecycles, while we continue to specialize in the delivery of comprehensive maternal and child health services to address this unmet need in our community.

Medical Services and Programs

Our Health Care Services Include:

  • Pediatric and Adolescent Health Care that includes well-child exams, immunizations, sick infant/child exams, school and sports physicals, and lead screening
  • Women’s Health Care (ob/gyn) that includes maternity care and delivery, Paps,and breast screening
  • Adult Health Care
  • Senior Health Care
  • Diagnosis and treatment of illness
  • Disease screening and prevention
  • Family planning (birth control)
  • Management of chronic illness such as asthma, diabetes and high blood pressure
  • HIV testing, counseling, and primary care

Our Support Services Include:

  • Counseling services
  • Linkage and referral to community programs and services
  • Linkage to substance abuse services
  • Family support services
  • Free books for young children through our Reach Out and Read Program
  • Free childbirth education classes
  • Financial counseling
  • Transportation assistance for those in need
  • Medication assistance for those who qualify
  • In-patient care at West Suburban Hospital
  • Ultrasound at West Suburban Hospital
  • Health education
  • Neighborhood outreach
  • Maternal/Child outreach
  • Parenting classes
  • WIC form completion
  • Asthma Care Training for children 6 to 12 years old and their parents

In order to ensure our services are affordable and accessible:

  • We accept many major health plans
  • We accept Medicaid and Medicare
  • We offer a sliding fee scale based on total family income and size to all individuals in need
  • We provide application assistance for Illinois KidCare and FamilyCare
  • Spanish speaking medical and support staff are available on-site
  • We provide health care and preventive services to all ages from infant to senior

Vision

PCC Community Wellness Center, in partnership with its physicians and staff, provides quality medical services to the medically underserved in the community.

Management Principles and Values

The management team is committed to promoting an environment of respect, trust, and candor. It is agreed that these interdependent values are essential to the team’s ability to further the organization’s mission of providing comprehensive health care services to the community, with a particular focus on the underserved.

  • Effective Communication - It is agreed that effective communication is essential. It must be timely, inclusive, and comprehensive.

  • Humor - Humor helps a team build a bond, relieve stress, and put issues and concerns in the appropriate perspective.

  • Teamwork - It is agreed that an organization is only as successful as its collective team. The dependence and reliance on each member of the team is recognized and valued.
