PCC is committed to creating an inclusive, safe, and welcoming space for everyone who walks through our doors. In addition, we offer a sliding fee discount program for patients who do not have insurance or who do not qualify for Medicare or Medicaid. Financial counselors are available to help determine your discounted payment for services. A social security number is not required to receive care. What does it mean to be a Patient-CePacifier Baby without pacifierntered Medical Home?

A Patient-Centered Medical Home includes the delivery of high-quality, cost-effective primary health care which is coordinated through your primary care provider (PCP) in a timely manner. As a Patient-Centered Medical Home, PCC uses evidenced based care and provides support for you to manage your health and healthcare.(Click here for more info.)

Why is it important to select a Primary Care Provider?

Selecting a primary care provider is a very important step for your health care. A PCP can give you a trusting, ongoing relationship with one health care professional and their team. Because a PCP looks at your whole health, they work as a part of a care team that coordinates your care across many settings.

Why should I make an appointment and how should I be prepared? 

Making an appointment increases the continuity of care with your primary care provider and decreases your wait time.

Always be prepared for your office visit, bring your photo ID, current insurance card, and any paperwork from a hospital or doctor you have seen recently. Remember to bring all medications you using to every visit. Copay is due at the time of service.

What if I need a referral?

We offer comprehensive primary care; many of your concerns can be addressed at PCC. If you need a specialty service that is not provided at PCC, you may require a written authorization based on the insurance you have.

Referrals require approval by your primary care provider. Know your insurance type. Some (including Medicaid and PPOs) need only a prescription. Others (such as HMOs, FHN, Harmony, etc.) need an authorization through your insurance company.

I lost my job and my insurance; can I still see the doctor?

Yes. PCC never refuses service to anyone due to inability to pay.

Please talk to a PCC financial counselor who can assist you in determining your eligibility for insurance plans such as Medicaid, All Kids, PCC's sliding fee discount program, or other payment options. PCC's sliding fee discount program is based on your family size and income.

How do I get my lab results?

Lab results are usually available on the MyPCC Patient Portal within a week. We may also call you to tell you urgent results. Please make sure we have your correct phone number.

How do I contact PCC for medical advice?

If you have a life threatening emergency, please call 911.

If you have an urgent question, please call us and ask to speak with a triage nurse. If a nurse can't answer right away, you can leave a message and a nurse will call you back as soon as possible.

If your question is not urgent, please leave a message with one of our staff members and we will return your call as soon as possible.

You can also use the MyPCC Patient Portal to contact your provider.

What is the fastest way to get my medicine refilled?

At your appointment, ask your provider for enough refills to last until your next appointment so that you do not run out.

If you do run out of medicine before your appointment, check your medicine bottle's label. At the bottom, it will tell you if you are already approved to get a refill. If you are approved, call the pharmacy or take your prescription bottle to the pharmacy and get a refill.

If no refills are left, or you are unsure, you can request refills in two ways:

Call the pharmacy to find out if you’re approved for refills; or use the MyPCC Patient Portal.

I need a form filled out, what do I do?

We are happy to fill out forms required for WIC, school physicals, job physicals, disability, etc. Please drop off the form requiring completion at least 3-5 business days before you need the form. Some forms may require a provider visit.

Does PCC have any quality measures?

PCC has many ways of measuring quality. Click here for quality metrics based on Healthy People 2020 goals. We can also provide other quality measures upon request.

What if I need to talk to a provider and the office is closed?

PCC is pleased to provide medical emergency service by telephone 24 hours a day, every day of the year. After regular business hours, you can reach an on-call provider who can handle your medical emergency by calling our regular office number.

For matters that are not urgent, please call during regular business hours. This includes completion of forms, test results, scheduling an appointment, referrals and any medical concerns that can wait until the next business day.
